Sharing the S.P.I.R.I.T. – Community Benefit & Health Improvement Plan, FY 2014-2016
The Community Health Needs Assessment of 2012 provided substantial information about the health status of our community. Through the Assessment process, a clearer understanding about our community’s health improvement opportunities emerged from the voices of those who live here, from secondary data sources comparing Carroll County to other communities, from targeted focus groups, and from many other sources with information specific to Carroll County, Maryland.
Sharing the S.P.I.R.I.T. is the title of the publication summarizing the Assessment process and outlining the implementation strategies proposed to impact our need areas and produce measurable results. It is the Community Benefit Plan for Carroll Hospital Center and the Health Improvement strategic plan for The Partnership for fiscal years 2014-2016.
Governing bodies of both organizations have provided outstanding leadership throughout this process, and will continue to monitor our efforts and progress through a formalized annual evaluation system. Evaluation data will be tracked in our system called Healthy Carroll Vital Signs, which will be updated annually, posted on this site, and reported at The Partnership’s annual public forum.
