How We Get Results

The Partnership achieves health improvement progress through Leadership Teams (previously called work groups) and Action Teams  formed around the Community Health Improvement Areas. It may also initiate collaborative efforts to address emerging health issues.

Community Health Improvement Areas are identified through the community health needs assessment and collaborative prioritization process. Experts and interested parties from each Area are invited to be on Leadership Teams to participate in community health improvement initiatives.

Leadership Teams use data from selected health indicators to measure progress in their Areas. The Leadership Teams then establish objectives and action plans to “move the needle” (improve indicator data).

Leadership Teams may form sub-groups known as Action Teams to better manage specific action plans.

Annual reporting, review, and recognition of progress takes place at our annual community health forum,  often known as We’re On Our Way.
